Easily accessed by medical personnel throughout the world.

The information contained in this history will depend on how extensive and how accurate the answers are given in the medSOS questionnaire. This questionnaire contains sections for each area subject to medical intervention.

You must fill out each box with your data, keeping in mind that the more information given, the more useful it will be. Of course you are free to complete the entire form or only the parts you consider necessary.

Keep in mind that omitting vital information only works against you, as you freely and willingly fill out the questionnaire and decide what you want to reveal or not, thus outlining the information the doctor will have about you.

We suggest that you calmly fill out and print this questionnaire. You may even request assistance from you family doctor.

Symptoms.


Your specialised simultaneous translator.

A list of potential symptoms such as fever, vomiting, headaches, dizziness…etc are made available to you.

These tools refer to the symptoms that your are experiencing at that very moment, in your own language. They will be read by the doctor in his/her own language, which if you are abroad, may not be the same as yours.

We provide you with a series of tables and charts with medical measurements.
This tool allows you to keep a record and analyze the progress of the measurements requiring your attention.

The measurements included in medSOS have been selected for being the most common and because you can measure them yourself (body mass index, blood glucose, blood pressure, pulse, cholesterol, temperature, paces per day.) through the various devices available in the market.

It is important for you to keep all your measurements updated by regularly inputting your new values. Keep in mind that this data may be useful for the doctor in making a diagnosis.
